2026 Master Entry Form

Please complete this online Master Entry Form by February 4, 2026. 

Use the Category List on our website (www.pimw.org/star-awards/) to decide what category your entry fits into. You will use the Category List to help fill in the “Category Letter & Number” below. You will need to create a title for each entry.

​IMPORTANT: The way you list your “Company Name” and “Entry Title” on this form is how it will be displayed on your awards and in marketing/promotional material. The way you list your entries on this form MUST match the ID Forms that you print off and attach to your entry.

Next, print and fill out the ID forms also on our website. These forms will be sent with your print entries.

Mail ID forms and entries to PIM’s office – 8011 34th Ave. S, Suite 150, Bloomington MN 55425 – by the February 4, 2026 at 12:00 PM. Submissions will not be returned.

Please notify PIM once you send your entries at starawards@pimw.org.

Your first entry is FREE! Each entry after that will cost $40. If you enter the same project into multiple categories, it is still $40 per entry.

How many total paid entries are you submitting?
To find the total cost = total # of PAID entries x 40
